The ZSFG Health Advocates is a program designed to improve patient care by bringing volunteers, social workers, and legal aid providers together to triage and help patients address social and legal needs that are barriers to good health. Volunteers are trained to use standardized social screening tools with participating families to help prioritize their needs and then help connect families with local resources.
