Volunteers use the Resource Flowsheets to effectively connect families with appropriate people, programs, and services to meet their identified needs. All families we screen are also given a social needs resource packet.

When a patient indicates they are suffering from food insecurity, the HA volunteer then works to address food needs for the day and/or at the end of the month.
If a patient or caregiver is looking for work there are multiple employment options and questions that volunteers can offer the families we serve.
If a patient or family indicates they or others in the household are undocumented, volunteers ensure that their needs are met and that their rights are respected.
Volunteers follow up with families to ensure resource connection and provide additional resources as needed.
