Impact

The HA program predominantly operates within ZSFG at two outpatient clinics, the Children’s Health Center and the Family Health Center, addressing healthcare barriers by focusing on social determinants of health and resource access.

In March 2020, the program underwent a transition to remote-only service delivery due to COVID-19 pandemic lockdowns. Subsequently, in April 2023, the program resumed in-person services, adopting a hybrid method.

Since July 2016, the Health Advocates have served over 7,236 patients!

HA patients are mostly children in Spanish-speaking households who are struggling with food access and housing.
